No provider client relationship
Accessing this website, reading its content, submitting an inquiry, requesting an appointment, or communicating through a general website form does not establish a provider client relationship with Cotallat Psychiatry or any clinician affiliated with the practice.
A provider client relationship is established only after appropriate intake procedures have been completed, the prospective client has been accepted for treatment, and a clinician has agreed to provide care.
Submitting an appointment request does not guarantee acceptance into the practice or the availability of a particular clinician, service, medication, or treatment.

Please avoid submitting unnecessary sensitive medical, psychiatric, or other health information through general website forms or standard email.
No method of electronic communication can be guaranteed to be completely secure.

Cotallat Psychiatry provides telehealth services only where permitted by applicable law and where the treating clinician is appropriately licensed or otherwise authorized to practice.
Cotallat Psychiatry currently provides services to eligible clients physically located in New York and New Jersey, subject to clinician licensure, clinical appropriateness, availability, and applicable law.
Clients may be asked to confirm their physical location at the time of a telehealth appointment.
A clinician may be unable to provide services if the client is physically located in a jurisdiction where the clinician is not authorized to practice.

Coverage, deductibles, copayments, coinsurance, prior authorization requirements, and other benefits are determined by your insurance plan.
Clients are responsible for confirming their individual benefits and coverage with their insurer.
